Further information

You will be a member of an integrated Occupational Therapy Service across South Gloucestershire Council's Community Care Department and North Bristol Trust Community Health, providing a consistent, equitable, and high quality user focussed service. You will link in with Health staff, Community Care staff and other agencies to support service provision to enable intervention that is responsive, timely and clinically effective.

You will be working with service users over the age of 18 that have physical and sensory impairments and Older People. You will be involved in supporting people to live at home through the assessment and provision of equipment and housing adaptations. You will also be involved in enabling them to live with maximum independence and dignity to prevent any unnecessary admissions to hospital or care homes and to facilitate timely and safe discharge from hospital.
