We currently have the opportunity for a Social Work Assistant to join our team. This is an exciting time to join the team following our recent Ofsted inspection, as we are rebuilding the team with a new management in post from 1st April 2017.
The 0-25 Disability Service works with children and young people eligible under the SEND code of practice, in receipt of an Education, Health and Care plan (EHCP). This may include children and young people in need, who need to be safeguarded and children who are looked after. Modelling good practice, the post holder will carry out the duties and responsibilities under relevant legislation, promoting the proper care, support, safety and welfare of adults at all times.
Under the supervision of the Senior Social Worker, the post holder will work closely with young people making the transition to adulthood. They will manage cases which present as straightforward, and are within the skill of the post holder to undertake. There will be advice and guidance from more senior staff on all casework, with regular supervision, and also support from Education colleagues and Occupational Therapy when required. Where necessary, there will be joint working with a Social Worker.
The post holder will work closely with Education and other colleagues within the 0-25 Service, to support the service user to monitor progress and agree changes to their Support Plan where required.
The post holder will be required to visit service users in their home and at educational settings. On occasion you may be required to deal with people whose behaviour is challenging.
The post holder will be expected to concentrate on older age young people and young adults within the 0-25 Disability Service, assessing their social care and health needs and to plan, implement, review and monitor action and outcomes.
This includes working within the framework of the relevant legislation, Care Act eligibility criteria, and safeguarding adults policies, and at all times working towards giving a greater choice, independence and social inclusion for the service user. The post holder will be required to have an awareness of mental capacity legislation, and operate compliantly within the Mental Capacity Act. You will review and monitor each Support Plan and the quality of the service provision, making changes in response to evolving needs and service principles. You will model best practice and, in line with legislation, policy and guidance, you will promote the wellbeing and dignity of service users enabling self-directed support and managing risk to ensure safety. You will where required, provide advice and guidance to service users and their carers on the use of personal budgets.
