Early Help ‘Open to review’ Co-ordinator (children with disabilities)

Limited Term Contract for 12 months

Summary of Post

An exciting opportunity has arisen for a new role in the 0-25 disability service, and specifically the Children with disabilities team, working closely with children and families to provide key elements of early help preventative work which complement and support the full range of services provided.

You will work with a number of children and young people and will undertake home visits, to coordinate implementation of care and support plans. You will act as the lead professional for young people with disabilities, aged between 0- 18, who have existing Child in Need plan, and who have stepped down to our ‘open to review’ (OTR) status. As such you will oversee the regular reviews of plans and ensure that needs are being met, and positive outcome achieved, and where appropriate, update information to ensure that any new needs are responded to at the appropriate level, including stepping cases back up to social workers within the team if and when appropriate.

The role will require working effectively within a multi-agency context and building productive relationships with colleagues from other organisations. Your role may also include completing transition assessments to identify pathways to adulthood, or providing EHCP contributions for young people with disabilities. All activities undertaken will include visiting various educational settings, family homes or places of service provision. You will need to be able to form positive relationships at all levels, and be willing and able to consult with young people, their families and or carers, professionals and service providers.

You will need to be able to organise and chair review meetings, prepare and share reports in a timely and professional manner, as well as be able to represent young people or the team/ service at a variety of internal and external meetings and panels as relevant to the families allocated to you.

The post holder will also be integral to the development of the role as part of the pilot which will evolve and develop to inform future planning and resource. You will be managed and supervised by either the 0-25 Senior Practitioner or the 0-25 Team manager, and will be supported by regular supervision, team meetings and training opportunities to ensure that you are practicing to the high standards expected within South Gloucestershire, by our young people, families and partners. You will model good practice and have knowledge and understanding of legislation and guidance which specifically relates to disabled children/young people and their families.

This is a 12 month limited term contract whilst this new role is piloted.

How you will make a difference:

Plan, chair and minute reviews of existing plans for disabled children and their families who are held on the OTR system. Ensure that review meetings are robust, informed by involved agencies, children and their families, and that plans have SMART targets which seek and evidence improved outcomes for CYP

Carry out assessment of social care needs relating to young people transitioning to adulthood, or to inform the EHCP, and ensure where appropriate assessing young people’s mental capacity, and identifying any deprivation of liberty and helping plan ‘next steps’ for adulthood

Take any necessary action in accordance with Safeguarding procedures in order to protect vulnerable children and young people.

Attend relevant panels and internal and external meetings in relation to young people and their families. Provide appropriate advice and guidance to others regarding various universal, targeted and specialist services, and where required share information to ensure the safety and wellbeing of children in line with Data Protection legislation and local authority policy.

Manage own caseload and ensure that records are completed to high standards, within agreed timeframes, and share with relevant parties having secured appropriate consent for activity.

Liaise and work with contacts from different sectors, including health and education, sometimes as part of a multi-disciplinary team, ensuring that communication is effective and that knowledge and best practice are shared.

Represent the Council’s interests ensuring that the Council’s position is advocated.

Maintain confidential reports and case notes, record care plans and risk assessments in accordance with professional and Council guidelines.

Selection Criteria

What we need from you:

Qualified at NVQ 3 or equivalent in a relevant discipline or working towards an appropriate qualification related to the provision of services to children and young people. Educated to 5 GCSEs level or equivalent (inc English and maths).

Considerable experience of working in a children and families service with vulnerable children, which should include an assessment function, service provision and experience of preparing written records and reports.

Effective verbal and written communication skills, and relationship building skills.

Ability to complete quality assessments, reviews and plans.

Awareness and experience of dynamic risk assessment whilst preparing for and performing a task, and ability to take appropriate action as a result of risks identified.

Experience and/or awareness of child protection issues.

Knowledge of relevant childcare legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.

A good level of general IT skills and the ability to use, or be trained in the use of, appropriate software packages.

Use of a vehicle: If the nature of the duties require you to travel on Council business on a regular basis, so you must either have a current driving licence and provide a car, or have access to appropriate means of travel.

If you use your vehicle on authorised Council business, you will receive a mileage payment based on the Inland Revenue Authorised Mileage Allowance Payment Scheme (AMAPS). The allowance is provided on the condition that your own insurance covers you for business use and indemnifies the Council against claim (including those concerning passengers) arising out of the use of the vehicle on official business. You will also be eligible for a car loan if you meet the criteria within the Council’s Car Loan Scheme.
