How you’ll make a difference

As a Social Worker with the Central team you will make a difference by being an integral part of our multi-functional provision in Adult Social Care, promoting independence, choice and wellbeing. Understanding what really matters to our families and their carers and have the ability to identify their strengths and help connect them to suitable resources and services. Working closely with our communities with the ability to understand local needs by working with individuals in a positive and responsive way.

What does an average day look like

This is an essential role within Adult Social Care, where on a daily basis you will meet and visit people and their carers to understand their needs and things that really matter to them. You will be helping them connect to suitable resources and services, providing a seamless service within a timely manner.

You’ll review and monitor care plans and the quality of service provision and respond to changing needs. This includes, develop and evaluate reablement plans as part of care packages ensuring programmes to support independence are implemented.

Be responsible to make important recommendations for a significant budget spend ensuring maximum value is delivered for the resources deployed.

You’ll be responsible for managing your own caseloads or specific projects to ensure specialist inputs are obtained and effective outcomes achieved.

You will be a skilled communicator, including liaising and work with contacts from different sectors, including NHS, partners and our community.

You will be able to take the necessary action in accordance with Safeguarding procedures in order to protect vulnerable people.


What we need from you

• You will hold a Degree in Social Work or equivalent (eg DipSW, CQSW, CSS) and be SWE registered

• You will demonstrate your knowledge of relevant legislation, regulations, codes of practice, processes and procedures.

• You will have experience of managing complex cases and demonstrate that you can apply excellent assessment skills in practice

• You must be excellent at time management so that you are able to work efficiently to meet the service requirements

• Preferably you’ll have some knowledge and Safeguarding experience.
