0-25 Transition Assistant
Summary of Post
Permanent Full Time (x1)
About the Role
You will be a member of the Transition Team within the 0-25 Disability Service. You will work with children and young people with a learning and or physical disability from age 13 -25 who have an Educational Health Care Plan (EHCP) or an allocated worker from social care. The Transitions team work with children and young people transitioning from Childrens to Adult Services and in promoting an individual in reaching their full potential across the four Preparing for Adulthood (PFA) Pathways.
Working in an outreach model your role is to promote the independence of a child or young person from year 9, with the aim of enabling them to reach their full potential both within their home and in the wider community under the guidance of a Transitions Facilitator.
You will work across the 4 key pathways to embed new services / practices for young people through the transition to:
• Gain skills and prepare for further education, employment, training, supported internships or volunteer work
• Be part of their local community and build friendships.
• Guidance and support in accessing and maintaining independent living, including travel training individually and travel training projects within schools.
• Guidance on staying safe, being healthy and connecting to health services where appropriate
You will be expected to record your work to provide evidence of the work completed whether one to one sessions, with the child/ young person and or their parent/carers, professional meetings, with schools and or social care. To be able to plan sessions and coordinate your working week, making changes accordingly and being able to be flexible. If you are involved in travel training there may be a demand for flexible working before or beyond working hours by agreement, to accommodate the needs of an individual of which time is worked in lieu. Comply with risk and safety plans and report any incidents or immediate risks to the facilitator you are working with.

The team is operates from the Kingswood Centre, but you will be expected to work within the community so driving is essential.

Main Duties
To provide individual support to a child or young adult (13-25) using a Person Centred Approach’
To contribute to the mapping and the maintenance of which services are available in the local community.
To provide relevant information to the Transition Facilitators and where applicable attend reviews and assist to manage expectations of families, children/young people, schools and other professionals.
Contribute to information and advice provided to children/young people, families, schools and other professionals within an individual pathway/s of the SEND Preparing for Adulthood.
To complete and maintain appropriate records.
To support a child/young person with travel requirements or be involved in a school travel training project.
The post holder may need to support or assist in the administration of medication for a child/ young person or their personal care needs in accordance with the individual assessed need.
To undertake support work where required in accordance with Social Care National Occupational standards and NICE standards.
To ensure the service of vehicles meet the required workplace protocols and checks which will include internal and external cleanliness.
Represent the Council’s interests ensuring that the Council’s position is advocated.
Selection Criteria

Essential
You will have training and experience of working in similar service areas.
You will hold a vocational qualification e.g.QCF / NVQ 2 or equivalent is desirable, you will be expected to acquire this if not already held.
You will show sensitivity and empathy in your work.
You will demonstrate knowledge of professional care groups and external contacts e.g. GPs, nursing services and emergency services.
You will be able to manage behaviour that challenges.
You will demonstrate knowledge of relevant legislation, regulations and codes of practice.
You will demonstrate knowledge of specialist equipment and an awareness of Manual Handling techniques.

If you use your vehicle on authorised Council business, you will receive a mileage payment based on the Inland Revenue Authorised Mileage Allowance Payment Scheme (AMAPS). The allowance is provided on the condition that your own insurance covers you for business use and indemnifies the Council against claim (including those concerning passengers) arising out of the use of the vehicle on official business.
