Salary or Salary Band: £37,890 - £40,876 pro rata

How you’ll make a difference

You will make a difference by being highly supportive to a team of SW and OT practitioners using and developing your own personal skills, values and personal qualities to support these practitioners to develop themselves personally and professionally enabling them to provide a resilient and responsive service which is strength based and focuses on development of independence to our customers.


What will you be doing?

You will work alongside nurse assessors in BNSSG (Bristol, North Somerset & South Gloucestershire CCG (Clinical Commissioning Group), attending CHC (Continuing Health Care) panel, representing the Local Authority in joined up decision making. Advocating for service users and ensuring eligibility of CHC funding domains are met. Having a sound understanding of the 12 CHC care domains and ensuring a thorough and comprehensive assessment of needs is completed. Opportunities for joined up working, thinking more broadly, presenting cases at CHC panel or joint funding panel, and will require autonomous decision making backed with evidence are all crucial elements to this role.

Our Central Team is a large generic adult social care team one of three teams in South Glos. That has approximately 40 staff and covers two South Glos. Localities. The team has many functions and is home to the CHC team which is a small group of SW staff who manage the CHC assessments for all of the three locality teams. As CHC SP you will work collaboratively with partner agencies Sirona and the CCG in ensuring that thorough CHC assessments take place jointly and will be responsible for the direct line management and supervision of SW staff within the CHC team.

Your role will include coordinating and allocating work and authorising assessments and plans completed by staff.

You will be a member of the Central management team which includes a Team Manager and a number of SW and OT Senior Practitioners.

You will provide advice and support to all team members.

You will support staff with managing complex cases taking account of levels of risk, issues of mental capacity and DOLS as required.

You will be balancing competing priorities and ensuring effective prioritisation and allocation of work, using the most appropriate resources.

You will be ensuring good quality assessments, care planning and intervention are completed meeting statutory requirements and within a strength based way.

What we need from you

You will hold a professional social work qualification and be SWE registered.
You will demonstrate specialist up to date knowledge of the service area and relevant legislation, regulations and codes of practice including equalities.
You will have proven ability to supervise and allocate work
You will have excellent organisation skills and the experience of managing complex cases
You will have well developed interpersonal and communication skills
