Summary of Post

This role is to support work to improve the health outcomes of overweight and obese children and young people (CYP) in South Gloucestershire. The main focus of the role is to support the delivery of Reach, tier 2 child weight management service. For more information on Reach visit www.southglos.gov.uk/reach

The post holder will use the Reach scheme of work to deliver group and 1:1 sessions. to families with children who are above a healthy weight in the community.

The post holder will be employed on a zero hour’s contract, working under the direction of the Healthy Weight Coordinator for Children, Young People and Families.

Main Duties

• Deliver the education parts of the Reach programme including nutrition and emotional wellbeing

• Deliver 1:1 and 10 week group programmes in community settings and schools (likely to include Thursday afternoons)

• Link with partners to ensure appropriate information sharing and joint working

• Maintain a record and comment form for work completed and sumbit these at agreed times

• Communicate effectively with children, young people and adults

• Facilitate group sessions for families to support positive lifestyle changes that encourage a healthy weight

• Feedback to the Healthy Weight Coordinator on the progress of individual referrals


Selection Criteria

Essential

• You will have a qualification in nutrition and have attended relevant training

• You will have experience of facilitating or delivering to groups with children and their families

• You will have an understanding of the needs of service users

• You will have good interpersonal skills, and be able to demonstrate well developed communication skills and ability to build relationships

• You will have experience of facilitating or delivering to a group programme

• You will have good recording and admin skills.

• You will be able to manage risk using information prior to working with families but also as situations arise.

• You will have considerable experience of working with customers with complex needs.

• You will have the ability to write clear concise reports with a good level of IT skills and the ability to use or be trained in use of specialist in house systems

• You will have knowledge of relevant legislation, codes of practice and support mechanisms such as child protection, lone working and communication channels with the Healthy Weight Coordinator

• Exemplifying the Council's values and behaviours.

Desirable

• A NVQ 3 or equivalent in nutrition
• Knowledge of health behaviour change
