Social Worker Assistant

Summary of Post

One permanent full-time (37 hour) post available



About the Role

You will be a member of the North Community Team and will work with adults, their families and carers who may require social care services. Roles will include assessing social care and health needs and to plan, implement, review and monitor outcomes. At all times you will work towards increasing choice, independence and social inclusion for your service users.

The North Community team consists of both social work and occupational therapy professionals. As a social work assistant you would be involved in assessing new referrals to the service. In addition you may be involved in facilitating discharges from Community Health Rehabilitation beds situated in local care homes.

The team is very supportive with a wide range of skills, knowledge and experience. You will have access to direction, support and supervision from a Senior Social Work Practitioner.

Main Duties

Carry out assessment of needs, providing advice and guidance to service users and their carers’, in order to establish reablement support plans and care plans where appropriate, ensuring that services are effective and meet legislation, regulations and codes of practice

Review and monitor care plans and the quality of service provision, responding to changing needs and renegotiating service provision where appropriate.

Manage own caseloads or specific projects to ensure specialist inputs are obtained and effective outcomes achieved.

Support the delivery of designated areas of work by coordinating day to day operations and contributing to developing projects and processes.

Raise awareness of a range of care issues and their implications and carry out risk assessments where appropriate, in order to ensure that needs are assessed and appropriate action is taken.

Develop and evaluate reablement programmes as part of care packages ensuring that programmes to support independence are implemented.

Liaise and work with contacts from different sectors, including health and the voluntary sector, sometimes as part of a multi-disciplinary team, ensuring that communication is effective and that knowledge and best practice are shared.

Complete and present records and reports in accordance with professional and council guidelines.

Take necessary action in accordance with Safeguarding procedures in order to protect vulnerable people.

Make recommendations for budget spend ensuring that maximum value is delivered for the resources deployed and contribute to budget management. This will ensure that the maximum value is delivered for the resources deployed.

Represent the Council’s interests ensuring that the Council’s position is advocated.




Selection Criteria

Essential

Experience in a relevant area of statutory or voluntary sector work in the health or social care field.

A relevant qualification e.g. NVQ Level 3 in health or social care or equivalent.

IT skills to use client data system.

Understanding of the needs of service users and good interpersonal skills.

Knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment

Ability to carry out risk assessments

Exemplify the Council’s values and behaviours


Desirable

Knowledge of the availability and scope of care funding

If you use your vehicle on authorised Council business, you will receive a mileage payment based on the Inland Revenue Authorised Mileage Allowance Payment Scheme (AMAPS). The allowance is provided on the condition that your own insurance covers you for business use and indemnifies the Council against claim (including those concerning passengers) arising out of the use of the vehicle on official business.


This is a permanent full-time (37 hour) post based a Badminton Road in Yate.
