Carers Support Centre (CSC) Bristol and South Gloucestershire provides a wide range of information, advice and support services for unpaid adult and young (children) carers.

A carer is someone of any age who provides unpaid support to a family member or friend who could not manage without this help due to; illness, disability, frailty, mental ill-health or substance misuse. Three out of five people in the UK will become carers at some point in their lives.

We have a vacancy within our Hospital Carer Liaison Team to work with local hospital Trusts to support carers and work with NHS staff to ensure that services are carer focused.

Hospital Carer Liaison Worker

22.5 hours per week (3 days, based on 37.5 hour working week)

Salary Range £23,354 - £25,903 per annum, pro rata.
The successful candidate will be based at the South Bristol Community Hospital working with health professionals to ensure that carers are; actively involved in discharge planning, recognised and valued as expert partners in care and included in decision-making.
You will; provide support to carers, act as an advocate, signpost to other services, liaise with Trust staff, local authorities and other agencies as required.
As part of a small dedicated and dynamic team you will be required to manage your own workload, be IT literate, maintain accurate records, produce reports and be able to engage with a range of professionals at various levels.
