Summary of Post


As Senior Special Educational Needs and Disability (SEND) key worker you will be part of an integrated multi-disciplinary service which is a part of The Education, Learning & Skills directorate.,
This post will be a part of a newly formed project team that will focus on improving the quality of Education, Health, Care Plans (EHCP) and quality assurance of provision within identified year groups. You will act as the Local Authority representative at Annual Review meetings and will be responsible for appropriate challenge to a range of providers whilst ensuring that the Young Person’s views and aspirations are captured and that the EHCP is specific, quantified and of good or outstanding quality.

You will work closely with members of the 0-25 SEND Casework Team; Educational Psychology Team; 0-25 Children’s Social Care; 0-25 Adults Social Care and a range of other professionals as appropriate to the individual cases for which you will have responsibility.

You will be required to attend and present cases to the weekly Annual Review Panel and will be responsible for ensuring the timeliness of decision making, and processing of decisions is within the statutory timescales as prescribed by current legislation

As Senior Case Co-ordinator you will be accountable for complex cases within the targeted focus groups of children & young people; complex cases may include; looked after children, cases where there are disputes with families and providers, where there is a high risk of school breakdown, young people who have been sectioned or where there are concerns about youth offending , post 16, home educated or out of area placements

Main Duties

You will model good practice and have knowledge and understanding of legislation which specifically relates to disabled children, young people and their families.

You will be required to work within statutory time scales to complete and review plans to the required quality standard.

You will provide advice and support to SEND Case Co-ordinators within the annual review team and to other colleagues across the service and contribute to and support the team development plan.

You will act as a point of contact for; schools, health, teams within the service and other agencies to provide advice and guidance on complex cases.

You will chair and attend annual reviews where there is a high risk of placement breakdown or a complex case within the targeted age groups. You can call upon support from Team Leaders/ Team Manager where required.

You will provide information and support around the Local Offer and will be expected to stay up to date with the Local offer itself.

You must be vigilant to safeguarding issues and must refer to a senior when you have any concerns about the safety or welfare of a service user.

Under the direction of the Team Leader SEND you will lead on tribunal case management up to hearing stage. You will also be required to lead on the formal stage of mediation on contested panel decisions

You will ensure that reliable ICT records and systems are developed and maintained for pupils with SEND. You will participate in audits and other activity under the service quality assurance framework.

You may be developed and act as subject matter specialist for others across the team; examples of specialism include; early years, post 16, electively educated children, autism, youth offending/prisons, children in care.

On occasion you may be required to deputise for the Team Leader SEND.


Selection Criteria

Essential

Degree or equivalent relevant work experience within sector

Knowledge of relevant legislation and statutory accountabilities of the service

Experience of working with a broad range of people including families, education, social care and health care professionals. In particular, experience of working with children and young people with disabilities

Have high level of interpersonal skills, be able to communicate findings, risks and decisions to service users, families, carers

Experience of leading multiagency meetings.

Experience of writing person centred, outcomes based plans/ report writing - able to write clearly

Skills to research local offer and ability to share it with families

Ability to use IT systems to keep records up to date and to use common software packages e.g. word, outlook

Experience of, or be prepared to work in accordance with the Council’s safeguarding policies and procedures

Must hold full drivers licence or be able to travel to various locations across South Glos.
