Full time post based within an adult care social work team working with a small team of Social Workers with a focus on Continuing Health Care cases.

To advise on and arrange care services including the assessment and review of care plans, in order to promote independence and deliver a high standard of personalised care.

Main Duties:

Attending and contributing to Continuing Health Care Assessments and reviews.

Assess, monitor, evaluate and review Continuing Health Care cases, care plans or reablement programmes, making recommendations for changes or adjustments to services as necessary.

Identify needs and arrange service provision ensuring that services are appropriate, accurate, targeted and well-organised and contribute to project planning meetings.

Take action to refer issues as necessary in accordance with Safeguarding Procedures in order to protect vulnerable people.

Provide advice, support and guidance to service users and their carers ensuring it is in line with Council and national standards

Selection Criteria

Essential:

Understanding of the National Framework for NHS and Continuing Healthcare and NHS Funded Nursing Care.

Experience in a relevant area of statutory or voluntary sector work in the health or social care field.

A relevant qualification e.g. NVQ Level 3 in health or social care or equivalent.

IT skills to use client data system.

Understanding of the needs of service users and good interpersonal skills.

Knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.

Ability to carry out risk assessments.

Exemplify the Council’s values and behaviours.


Desirable:

Experience in Continuing Health Care work.

Knowledge of the availability and scope of care funding

Use of a vehicle: If the nature of the duties require you to travel on Council business on a regular basis, so you must either have a current driving licence and provide a car, or have access to appropriate means of travel.

If you use your vehicle on authorised Council business, you will receive a mileage payment based on the Inland Revenue Authorised Mileage Allowance Payment Scheme (AMAPS). The allowance is provided on the condition that your own insurance covers you for business use and indemnifies the Council against claim (including those concerning passengers) arising out of the use of the vehicle on official business. You will also be eligible for a car loan if you meet the criteria within the Council’s Car Loan Scheme.
