As a Social Worker you’ll make a difference… providing support to vulnerable members of the community.

• Delivering genuine people-centred, strengths-based support, with a focus on what those we work with tell us will make the biggest difference to them.

What does an average day look like?
You will carry out assessment of the individual’s needs, providing advice and guidance to the user, carer and family in order to establish care plan, ensuring that services are effective and meet legislation and codes of practice. As a social worker you will review, monitor and record care plans, responding any change of needs and renegotiation service provision where appropriate. The assessment of needs is key, to raise awareness of a range of care issues and their implications and carry to out risk assessments to ensure that appropriate action is taken. You will liaise with contacts from different sectors to develop and evaluate re-enablement programmes as part of care packages to support independence.

What we need from you:
Degree in Social Work or equivalent (e.g. Dip SW, CQSW, CSS)
Part or Full registration with the HCPC.
IT skills to use client data system.
Have good interpersonal skills, be able to carry out assessments and communicate findings and risks to service users, families, carers
